In view of the lack of response to my earlier proposal, I now intend  to undertake an extensive redraft of this article. I shall, as far as possible,  retain the existing material concerning medical CBA while adding a substantial amount of additional material concerning its theoretical underpinning and its wider application.
I propose also to  replace the opening statement with a more general summary.

Proposed redraft
