I wrote an email to the Kentucky Geological Survey and asked for permission to use this diagram from their website. Here is their response which granted permission:

Mr. Beychok,

Permission for this type of use is routinely granted, with the stipulation that both the Kentucky Geological Survey and Stephen Greb, who is the artist, receive credit for the image.

I can write you a letter granting this permission. Do you need a paper copy mailed to you or will an electronic copy on UK / KGS letterhead?
